April 30: The following items need to be RECEIVED in the IMPACT Office:

May 14: The following items need to be RECEIVED in the IMPACT Office:
· ALL Student Forms
· Boy & Girl Rooming Lists
· The Indemnity Form
· Payment for balance due
For EACH STUDENT
1. Read a copy of the "Impact Rules for Students."
2. Fill out and sign the "Student Form."
Youth minister: Once you have registered, please fill out the top portion of each student's form online. (Click Here) Print the forms from there and then get signatures. Please verify the student's, the parent's, and the pastor's signatures before mailing.
3. Complete the "Student Medical Waiver." (3 pgs)
Remember: A completed Medical Waiver is required to attend IMPACT.
For EACH TEAM LEADER
1. Read a copy of the "Impact Rules for Team Leaders."
2. Fill out and sign the "Team Leader Information Form."
3. Fill out and sign the top portion of the "Recommendation Form." (2 pgs) Have your pastor fill out the bottom section of the "Recommendation Form."
