IMPACT FORMS
In order to provide a safe and secure environment for our Impact students, each registering Church, Team Leader, and Student must complete the forms included below. Please use the following summary as a guide for returning your forms on time.
FORM DUE DATES
The following forms must be RECEIVED in the Impact Office by their respective due dates.
April 30, 2010

For each STUDENT
- Read a copy of the "Impact Rules for Students."
- Fill out and sign the "Student Form."
Youth minister: Once you have registered, please fill out the top portion of each student's form online. (Click Here) Print the forms from there and then get signatures. Please verify the student's, the parent's, and the pastor's signatures before mailing.
- Complete the "Student Medical Waiver." (3 pgs)
Remember: A completed Medical Waiver is required to attend IMPACT.
For each TEAM LEADER
- Read a copy of the "Impact Rules for Team Leaders."
- Fill out and sign the "Team Leader Information Form."
- Fill out and sign the top portion of the "Recommendation Form." (2 pgs) Have your pastor fill out the bottom portion.
- New TLs: Complete the forms related to background checks. Click Here (8 pgs)

All form ORIGINALS must be RECEIVED in the office by their respective duedate. Please mail them to the following address:
